Covering letter on company letterhead with company stamp/seal and also mentioning complete address of the Invitee.

Original insurance

Return Confirmed ticket.

Original Work Invitation in Work format attested by OVIR in Ukriane(Please note that Business and Work Invitations have different Formats).

Registration Certificate of Ukrainian Company (Without this, Visa will not be processed).

Personal Bank Statement(3 months).

Authority letter

Prior Appointment has to be taken for submission of application even if the passenger is coming. For appointment first and last page of passport copy and the Original Invitation Letter required.
